Medicare Physiotherapy Sessions
If you are living with a chronic condition, you may be eligible for Medicare subsidised physiotherapy sessions under a Chronic Disease Management (CDM) Plan.
What Is a CDM Plan?
A CDM Plan is organised by your GP for patients with a chronic medical condition that has been present for six months or longer.
This plan allows you to access up to 5 allied health sessions per calendar year that are partially subsidised by Medicare.
These sessions can include physiotherapy.
Are CDM Sessions Free?
No. CDM physiotherapy sessions are not free.
Medicare provides a rebate towards the cost of each session. This means:
· You pay for your appointment at the time of your visit
· Medicare rebates a set amount back to you on the spot

How Do I Get a CDM Plan?
To access Medicare subsidised physiotherapy sessions:
1. Book an appointment with your GP
2. Discuss your condition and ask you GP if you are eligible for a Chronic Disease Management Plan
3. Your GP will prepare the plan and provide a referral for physiotherapy
o You are free to choose your physiotherapy provider
o If you would like to attend Movéo Physio, ask your GP to address the referral to Movéo Physio in Wentworthville.
4. Bring your referral and your Medicare card to your first appointment
Without a valid GP referral, Medicare rebates cannot be processed.
What Conditions May Be Eligible?
CDM Plans are typically for chronic conditions such as:
· Chronic back or neck pain
· Osteoarthritis
· Diabetes related musculoskeletal issues
· Long term shoulder, knee, or hip pain
· Other ongoing medical conditions lasting longer than six months
Your GP determines eligibility.
